Purpose of Telehealth
Telehealth involves the delivery of healthcare services using electronic communications, including assessment, diagnosis, consultation, treatment, and patient education. By using Puri's telehealth services, you consent to receive healthcare services via telehealth technology.
How Telehealth Works at Puri
Our telehealth services include asynchronous (store-and-forward) consultations where you submit health information through our online intake assessment, which is then reviewed by a licensed healthcare provider. Depending on your treatment plan, services may also include synchronous (real-time) video or phone consultations.
Your provider will review your medical history, current medications, symptoms, and other relevant information to make clinical decisions about your care.
Benefits and Risks
Benefits of telehealth include: improved access to medical care, convenience of receiving care from home, reduced exposure to communicable diseases, and ability to receive care outside of traditional office hours.
Potential risks include: limitations in the provider's ability to perform a physical examination, potential for technical difficulties that may interrupt the consultation, and the possibility that the provider determines telehealth is not appropriate for your condition and may recommend an in-person visit.
Your Rights
You have the right to: withhold or withdraw your consent to telehealth services at any time; request an in-person consultation instead; be informed of the qualifications of the healthcare provider; receive the same standard of care as an in-person visit; and have your health information kept confidential as required by law.
Prescriptions and Treatment
A prescription is not guaranteed as a result of your telehealth consultation. Your provider will make treatment decisions based solely on their clinical judgment and your individual health profile. If your provider determines that treatment is not appropriate, you will not be charged for the consultation.
Compounded medications prescribed through our platform are not FDA-approved and are prepared by licensed compounding pharmacies. Your provider will discuss the benefits and risks of compounded medications as part of your consultation.
Emergency Situations
Telehealth services are not intended for emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Our telehealth platform is designed for non-emergency consultations only.
Consent
By completing an intake assessment on our platform, you acknowledge that you have read, understand, and agree to this Telehealth Informed Consent. You confirm that you are voluntarily choosing to receive healthcare services via telehealth and that you understand the benefits, risks, and limitations described above.
